Re-registration Form
Please print out and complete this form if you voluntarily suspended yourself from the register by making a statutory declaration and you want to start trading again. You can only rejoin the register this way if you have been voluntarily suspended for less than three years. If you have been voluntarily suspended for more than three years please contact the HAC for details on how you can start trading again.
